Option1) Call us at (248) 351-0101 to request a registration form.


Option 2) Email your fax number to recieve a registration form via fax.


Option 3) Download a registration form.

The registration form is a PDF file. To open the file, you will need Adobe Reader,
which you can download here.

Please note: classes are filled on a first-come first-served basis.
